Run the full application
The Capvera Business Funding Application, every field, every time. This is the form that goes out to funders — a section left blank here is a question a senior advisor has to go back and ask the merchant for later, after the moment has already passed.
What the application covers
Seven sections. Every one of them matters.
Walk the merchant through it in order — most of it they can answer without looking anything up. The banking and processing sections are the two they'll need to pull up a statement for.
Basic Business Information
Legal business name, DBA, structure, EIN, date started, industry/NAICS code, and contact details. This is what makes the file searchable and verifiable — a typo here follows the file everywhere.
Funding Request Details
The amount requested, the purpose of the funds, and the desired term or repayment preference. Get the purpose specific — "working capital" tells underwriting nothing; "new equipment, $40k" tells them everything.
Primary Owner / Principal
Full legal name, title, date of birth, ownership percentage, SSN, and personal contact and home address. This must match the driver's license exactly, or the file gets kicked back for a mismatch.
Financial & Banking Information
Primary bank, account type, account and routing numbers, average monthly revenue, and any liens, judgments, bankruptcies, or outstanding business debt. This is where an existing position gets disclosed — never let the merchant leave it blank.
Merchant / Processing Information
Primary processor, monthly processing volume, average and max ticket, refund policy, and how sales are split — online, in person, over the phone. Skip this section for a merchant with no card processing; never skip it for one who has it.
Required Documents Checklist
Driver's license (front and back), voided business check, bank statements, and processing statements — each with its own upload field. This syncs straight into the Documents tab in Go High Level the moment it's submitted.
Authorizations & Certifications
The merchant reads the authorization disclosure, checks the box, prints their name and title, and signs. Submitting the form is the merchant authorizing Capvera and its partners to review everything above for funding consideration.
A partial application produces a partial file.
The two fields that get skipped most are the owner's SSN and the existing debts section — both because the merchant hesitates, not because they don't know the answer. Reassure them, explain why it's asked, and come back to a skipped field before you hang up. Do not send it to the senior advisor incomplete.
